Raising a Concern

At UPMC Aut Even Hospital, we take all feedback seriously and are committed to managing and investigating any concerns raised. Understanding why patients feel the need to complain helps us identify what isn’t working well and make meaningful improvements to the quality of care and treatment we provide.

If you are unhappy with the treatment or care you are receiving, we would encourage you to raise your concerns as soon as possible, preferably to senior member of staff on duty at the time. If, however, you remain dissatisfied please complete the form on this page.

Please note: you may submit a complaint on behalf of someone else. However, we will have to ask for permission/consent from the person involved (if they are aged over 18 and have capacity) to investigate the issues raised. If you are filling out this form on behalf of a patient of UPMC Aut Even Hospital, please complete the relevant fields on the form.
